If you have no insurance, we also welcome you to the clinic and accept all forms of payment. New Patients will need to fill out forms and provide insurance information in the clinic. PLEASE BRING YOUR INSURANCE CARD ON YOUR FIRST VISIT TO THE CLINIC. If you have more than one insurance, we need information for both (e.g., Illinois HFS and BCBS).
